Report of the PRESIDENTIAL COMMISSION
on the Space Shuttle Challenger Accident
In late 1982, Marshall Safety,
Reliability and Quality Assurance engineers reviewed test and
analysis results and determined tha the case joints should be
reclassified as Critically 1 (not redundant). This form was signed by
Maurice (Bud) Parker, a local Thiokol Reliability engineer, beginning
the process of management approval of the Criticality change.
